K-8 Registration

For new families, please be sure to fill out the inquiry form and set up a tour of Mount Calvary Lutheran before registration. After your tour, we welcome you to begin the registration process.

Step 1: Registration Form

This online application will ask for the information we need to get you fully set up in our online FACTS student information system that we use to track grades, share information with parents, keep records, pay tuition, purchase hot lunch, etc …

Step 2: Academic Record Request

A record request is made to obtain a copy of a student’s academic records from their current school.  Please complete and sign the form below and return it to the school office so that we may request these records.

This gives an overview of where each child is currently at in their learning & development and makes us aware of any special academic needs that may exist.  This step is in place because we want to make sure that every student we enroll at Mt. Calvary is being set up for success.  We want to make sure we have the staff and resources needed to ensure a student is able to thrive with us.  

Step 3: Application Review

Once both the parents and Mt. Calvary staff feel ready to move forward with enrollment, a letter will be sent to the family informing them that the application for enrollment has been accepted.  

Step 4: Hold Student’s Seat

A non-refundable $100 early tuition payment will be required at that time to officially hold a seat in a classroom (unless the family is part of the Wisconsin Choice Program.)
